Hey everyone! Sorry in advanced for the long post but i want to give as much info as possible! This is kind of an update from my previous post as well as more questions So this is my girl Jinx. She is a 7 year old leopard gecko. Approximately 2 months ago stopped eating. For about 4 weeks refused food and was very lethargic but still maintaining weight at 68 grams. She finally ate for me at the end of week 4 about 5 crickets (she has not eaten since, 3 weeks ago). We had pulled blood at that point and gotten the results (last picture) and determined liver. We started her on lactulose until we recieved milk thistle extract. On the lactulose i noticed her perk up more and in general have more energy but no luck with her appetite. Once i received the milk thistle extract i swapped over to that however she was very resistant to the point of holding it in her mouth and eventually shoving it back up and out her nostrils. She did this maybe twice? Which had then caused inflammation in her nostrils so we stopped that medication and went back to lactulose. Throughout all of this energy was slowly declining again, not quite as bad originally but still staying in one place with slitted eyes and occasionally changing areas. However now she is starting to lose weight. When i first started the lactulose we were holding steady around 70 grams. We are currently down to 66 grams and dropping. With her hatred of force feeding and a potential for fatty liver after brumation i had held off on force feeding her up until now since she was maintaining weight. 2 days ago i was able to get about 0.5ml of critical care omnivore into her before she lost her mind. I was going to attempt some more tonight. Today we repeated her xrays and were slightly concerned her liver was enlarged but we dont have a ton of reptile comparisons. We decided to throw our ultrasound on her. Now mind you we have zero experience with reptile ultrasound so my doctor wanted me to reach out to all of you to see if these things are normal or not. The first couple pictures were caudal abdomen. We werent sure if this was anything concerning as the object within the gassy area was slightly hyperechoic and it looks like theres a circular tissue around it. The second area which I labeled with a question was more cranial abdomen. Theres a circular tissue area on the right side of the picture. We weren't sure if this is normal stomach anatomy or if theres a possible mass coming off the liver. I appreciate all your time if you've read this far thanks in advance!

For me that looks like a follicle, did you check the caudal abdomen?
I think Cordelia Corpetmedvet Bracht mentioned follicular stasis/preovulatory dystocia in your other post. Everything about this screams follicles to me, but to be honest I cannot see anything on the x-rays, as taking a photo of a monitor of an x-ray is rarely good enough to say anything but "snowstorm at night", especially once facebook has lowered the quality even further.
Liver parameters are wonky with folliculogenesis, too, and I am not entirely sure what the lactulose is supposed to do (much) in this case, I think if I tried to get one thing in the milk thistle extract would be more important, and maybe glucose and vit B complex infusions, possibly amino acids if available, and that would help the liver tremendously, besides the fact that injections are easier to administer in an uncooperative patient than oral stuff ?
Try to find the ovaries in ultrasound, and there is always the option of exploratory laparotomy (with castration in case follicular stasis is found - or removal of liver mass should that be one).
